I don't know whether the exams I have had were similar to yours, but I've never had a physical without a male part exam, complete with the "turn your head and cough".If you are uncircumcised, I think it's great that your doc would check to see if it works properly. I'd tend to think that most docs don't/won't do that unless you have specifically noted that you have a concern there.

The turn your head and cough is to check for hernia. Just had that done last week. A careful rolling of the testicle to check it for lumps is important, since the most frequent time of life for cancer there is teens and 20's. I wrote elsewhere on here years ago about getting an ultrasound of my scrotum because of a place that turned out to be a calcium deposit.
When I was in 1st grade and 6th grade (1960's and 70's) there was a more thorough exam of the parts. Not abusive, but probably some study about variations. Retracted the foreskin, gripped the head and one pull to see length. One squeeze of the shaft, then wrote something down. The first grade time also gently squeezed each testicle asking if it hurt at all, and if so, to point to where.
