Is it common to have some kind of fetish about doctors and/or medical exams? How do you explain this to an actual doctor without giving offense? Does it mean you're not straight if you get excited?

There are certainly plenty of fantasy stories and videos like that. :) But I don't think there's much chance of an actual doctor doing anything on those lines--it would be considered unprofessional and if found out, there'd be no hope of practicing medicine again, I'd imagine.
I don't think it's as simple at any time as just gay or straight, and especially if there's a fetish involved as well.

It's called "medfet" by some & is VERY common among both genders, who participate both as play doctors and play patients. Some have their own medical equipment to act out play exams. They have their own ways of finding participants to examine them or be examined by them. Many people of both genders are into enemas. You DON'T explain it to a doctor. Docs can probably sense if the experience gets you off. If you get excited, they don't think much of it. They shrug it off as a natural reaction to being touched or exposed. They think it's normal and have seen it thousands of times. regardless of doc/patient gender.
Yes, some in the medical profession even have medical fetish as well, but they remain as professional as possible to keep their jobs. After all, they are only human too.
