Being in the water generally lessens the amount of sun exposure, but you're still being exposed. I'd suggest waterproof sunscreen if you're planning to spend and significant amount of time outside in the water.

I generally spend most of the time with the majority of my body submerged or completely underwater so I was thinking about it from a slightly different perspective.

Re-apply it often....even if you tan well.

Most of my time swimming is in the ocean, I am at the beach almost every other day during the summer. I dont really like wearing lotion cuz of the environment. Could I just wear something with no spf but protects against skin disease or there is no such thing.

Well, some of the old things like coconut oil have next to no SPF - but that means you get no protection from the sun.

Yeah... you can wear long sleeve shirts, pants and a hat. That'll protect ya.

It would be pretty damn hard to swim and surf with all that on.

When scuba diving in warm water, people often wear a rash guard, which is thin material that covers most of your body, for protection against sun and nasty stinging things. It might look a bit odd if you just swam in it...so get a surfbord and look cool.
