That's correct - for AIDS they take blood and look to see if you have antibodies to the HIV virus - you only have these antibodies if you have the virus. For other STDs they may look for antibodies or for the pathogenic organism itself.

I was meaning in the blood. A pathogenic organism is a bacterium or protozoon or virus or fungus or whatever that causes a disease. Sometimes these are easy to find in the blood, and sometimes they are not.
With some STDs (not AIDS) it is indeed helpful to look at your genitalia, and a doctor would normally do so. For gonorrhea they will take a urethral swab, best not thought about.
