I would try an antifungal cream for a week to see if it helps. If it doesn't, then I would try an antibacterial cream. If that doesn't work I would try a cream for allergies.

You should also make sure your washing properly.
Another cause could be any detergent or something you use to was your clothes. As your penis head is sensitive to just about everything, you should take care to make sure what it comes in contact with.Could this be Balanitis?

well iv been using the avenno this week and it seems to be working but slowly should i keep using it? should it be taking like 2-3 weeks to get better?and it seems that the only time it looks like it is getting better in when the cream is on it

If it's still swollen then I wonder if you should try another doctor? Something seems wrong - perhaps a deeper bacterial infection that will need antibiotics, or perhaps an allergy.
