Whats this rash

@whitemale29 It's difficult to see detail in your image. If you look closely, do you see any burrows or trails under the skin? I would suspect a mite such as scabies. Any doctor should be able to treat you, but a dermatologist would probably see things like this more often than a family doctor would.

When I saw the "really itches", I thought of scabies too. That itches very powerfully, especially at night. However there are other possibilities, such as pubic lice ('crabs'). and fungal infections ('jock itch').
Scabies mites and pubic lice can be got rid of with creams you can get at a pharmacist. Some creams will kill both. For fungal infections you need a different cream, an antifungal one--anything for tinea/athlete's foot/ringworm should do. Keep using the cream long after the symptoms have gone, to make sure they don't come back. Sexual partners should also be treated, and underclothes, sheets, towels, etc thoroughly washed.

@whitemale29 Did you ever go to the doctor? What was it?

Apply an over-the-counter hydrocortisone cream to the influenced zone if the rash is exceptionally irritated and causing distress. Calamine cream can likewise help assuage rashes from chickenpox, poison ivy, or toxic substance oak. Clean up. This can alleviate the irritation related with rashes from dermatitis or psoriasis.
